What to Complete

When you schedule your first appointment, we email a secure link to your new-patient intake forms. Complete them online at home in about 10 minutes and arrive at your appointment ready to go. The forms cover the standard information any dental office needs: medical history, dental history, insurance details, emergency contacts, and HIPAA acknowledgment.

Medical History

Current medications, allergies, medical conditions, prior surgeries. If you don’t know exact dosages of medications, bring the bottles or a photo of the labels.

Dental History

Prior dental work, dental concerns, goals for care. If you don’t remember when you last had an X-ray, we’ll take new ones.

Insurance Information

Carrier, group number, policyholder details. Bring your insurance card too, if you aren’t sure about your coverage, we’ll verify it before treatment.

Consent Forms

HIPAA privacy acknowledgment (standard form required of all dental offices), consent for treatment for the exam and cleaning portion of the first visit, and emergency contact information.

Don’t worry if you don’t have all the information. Fill in what you know. The forms are there to help us care for you well, not to trip you up on details.

How to Submit

Online (Recommended)

Complete the secure, HIPAA-compliant forms from the link we email when you schedule. Takes about 10 minutes. The secure link is tied to your specific appointment and expires after submission. Check your spam or promotions folder if you can’t find it, or call (208) 344-6300 and we’ll resend.

Email

We can email forms to you in advance on request. Complete them at home and bring them to your appointment, or email them back before your visit.

In Office

Paper forms are available at the front desk. Plan to arrive 15–20 minutes early if you’re filling out forms in our office to avoid shortening your appointment.

What to Bring to Your First Visit

  • Photo ID, driver’s license or government-issued ID
  • Insurance card, even if you submitted information online
  • Medication list, or bring the bottles / a photo of the labels
  • Records from previous dentist, if you have paper or digital records; otherwise we can request them with your written permission (typically takes 1–2 weeks)
  • Questions and concerns, write them down so you don’t forget during the appointment

Updating Your Information

For existing patients, we ask at each visit whether anything has changed, medications, medical conditions, insurance. If something major changes between visits (new medication, new diagnosis, insurance change), call us before your next appointment so we can update your records in advance. New medications especially can affect dental care; a quick call saves a surprise during the appointment.

Records From a Previous Dentist

If you’ve seen another dentist recently, we can request your records with your written permission. This saves retaking X-rays and lets us see the full history of work done. The request typically takes 1–2 weeks to process. You can also bring paper or digital records yourself if you have them.
